Step 1.

Information. Here is a thorough overview of the PHMEF Grant
application process. Read through the information regarding
process, procedures, requirements, and expectations
before proceeding with the application.
Step 2.
Please find the online Grant Application. After filling out all necessary and required fields, please print the information by navigating to the "File" drop-down menu at the top of your browser and selecting "Print."We also recommend that you save the document to your hard drive by navigating to the "File" drop-down menu at the top of each page and selecting "Save As..." After the "Save As..." window appears, be sure to find the "Save as Type:" option at the bottom of the window and choose "Text File.txt" You may then choose a name and location to save your work under. Please note: pages must be saved individually off the web page, then may be manipulated as desired.
REMINDER: please remember to have all required SIGNATURE FIELDS filled out after printing your document.
